Social Worker 


     Responsible for consultation and direction of social services in the Outpatient Psych and Partial Psych Unit. Conducts individual and group therapy while maintaining a caseload.  Completes psychosocial assessments, daily case notes, treatment planning and updates while working with the multidisciplinary team.  Responsible to work positively with the hospital team from patient admission to discharge.  May be responsible to lead treatment team meetings.  Responsible for maintaining up to date records on the patient. Maintains performance improvement activities within the department and participates in CQI activities. Demonstrates support of the Medical Center’s mission, values and vision in all actions and interactions with employees, patients, medical staff, guests and visitors through use of STAR key words: Service, Teamwork, Accountability/Attitude, Responsiveness



Qualifications

  • Current State Clinical Social Work Licensure.
  • Two (2) or more years of previous experience in all aspects of social service, with an emphasis in psychiatric work preferred.
  • Basic computer knowledge preferred.
  • Must be able to communicate in English verbally and in writing.
