Acute Rehab Clinical/Community Liaison - Full Time

Acute Rehab Clinical/Community Liaison
The Rehab Clinical/Community Liaison is responsible for generating and accepting referrals, completing the pre-admission screen in accordance with CMS regulatory requirements on all patients referred for acute rehabilitation services, as well as the duties related to the case/care management of all patients admitted for acute rehabilitation services. Participates in quality and process improvement in the department during all phases from planning to implementing and sustaining efforts.
The clinical/community liaison communicates the features, benefits, and competitive advantages of the acute rehabilitation unit. Demonstrates support of the Medical Center’s mission, values and vision in all actions and interactions with employees, patients, medical staff, guest, and visitors.
Licensed healthcare clinician or licensed social worker with current Utah State license. Registered Nurse (RN) strongly preferred.
- BLS Certification (CPR) issued by American Red Cross or American Heart Association AHA Healthcare Provider is required. Uploading a copy of your current BLS card to your application is strongly preferred.
- 2 - 4 years of healthcare experience.
- Able to communicate effectively in English, both verbally and in writing.
- Basic computer knowledge.
Acute Rehabilitation experience.
